Submitting the form on this website sends a request. It does not reserve or confirm a slot. Your appointment exists only once our team confirms the date and time with you.

Emergencies

This website is not monitored around the clock. If you have severe pain, heavy bleeding, swelling that affects breathing or swallowing, or a facial injury, seek urgent in-person medical care rather than waiting for a reply.

Accuracy of the details you give us

Please give us a phone number we can reach you on. We cannot confirm an appointment if the contact details submitted are incorrect.
